In order to best serve you, we would ask that you complete the following documents prior to coming for your initial intake appointment.

RCADS (Parent Version)

Please complete prior to intake appointment.

Vanderbilt (Teacher Version)

Please complete prior to intake appointment or follow-up appointments addressing ADHD symptoms. Feel free to give to as many of your child's teachers.

RCADS (Youth Version)

Please complete prior to your intake appointment
